We collect your name, phone number, email address, and school so we can arrange your placement. This information is shared only with program staff and the site liaison who reviews your request. We keep request records for program reporting as required by our funding agreements.
What we collect
Your full name, phone number, email address, and high school. We do not ask for medical, Social Security, or other sensitive information.
Who sees it
Northern WV AHEC program staff, and the liaison for the hospital and department you choose.
